Concussions, a common form of Traumatic Brain Injury (TBI), can result from direct head trauma, such as falls or accidents, as well as rapid acceleration-deceleration of the head, often seen in whiplash injuries. Contrary to common misconceptions, most concussions (90%) do not involve loss of consciousness.

Even so, mild concussions should not be underestimated. Brain injury specialists stress that there is no such thing as a “minor” concussion, emphasizing the significance of proper assessment and treatment.

Common symptoms of TBI/concussion include:

Brain imaging studies using MRI and CT scans are typically not useful for diagnosing concussions, as they seldom reveal significant changes and may unnecessarily expose individuals to radiation. Unlike structural defects, concussive injuries are often metabolic and microscopic, not readily visible on most neuroimaging.

The pathophysiology of a concussion is complex, involving functional disturbances and neurochemical events that disrupt neurologic function. Axonal injury, electrolyte release, neurotransmitter changes, glucose metabolism shifts, decreased cerebral blood flow, and mitochondrial dysfunction contribute to the injury’s intricate mechanisms.

Traditionally, rest was the recommended treatment for concussions. However, recent research suggests that extended rest, beyond 48 hours, may not be beneficial and could potentially hinder recovery.
